NDHU epidemic prevention and control level for Coronavirus Disease 19 (COVID-19) has been raised to level 1. All student clubs and associations guided by the Student Activities Division need to cooperate with risk assessment measures. Please refer to the announcement of the Student Activities Division’s online antiepidemic zonefor the application procedure. It also applies to any student association(such as department student associations)

Operational instructions:
Before applying for funds of extracurricular activities/club classes or for borrowing equipment and facilities from Student Activities Division for activities, all student clubs or student associations should submit the risk assessment form along with the proposal for review 10 days before the activity. The application, if not up to the standard, will be rejected for further adjustment. If the application is approved, please make sure to reinforce epidemic prevention measures and submit ”Access Record of Club Activity, COVID-19 Prevention by NDHU” after the activity.
The Student Activities Division should keep the original approved risk assessment form, and provide its copy to the applicant. Please show the copy when borrowing equipment and facilities for activity.
