條件:本校學生
地點:學活園區1樓川堂
費用:新台幣$700元整(請自備零錢)現場報到時繳費

本校依據學校衛生法第 8、9 條、教育部學生健康檢查實施辦法及東華大學學生健康檢查實施要點，辦理新生健康檢查及填寫新生健康問題評估表，所蒐集之資料將作為本校學生健康管理及衛教辦理之用，除因履行輔導、教學義務，或法律規定應提供外，將遵守個人資料保護法之規定，一律予以保密。

 Dear Freshmen and First-Year Graduate Students,

Attention all students who have not yet completed their new student health check-up:

To avoid multiple trips to the hospital, our office will be hosting a third health check-up session. We encourage all new students to take advantage of this opportunity.

Health Check Date: Saturday, March 14, 2016, 9:00 AM - 10:00 AM(No waiting beyond the deadline)

Eligibility: Donghua University students

Fee: NT$700 (exact change required) - Payable upon on-site registration

In accordance with Articles 8 and 9 of the School Health Act, the Ministry of Education's Implementation Measures for Student Health Examinations, and Donghua University's Implementation Guidelines for Student Health Examinations, the university conducts new student health examinations and collects New Student Health Assessment Forms. The collected data will be used for student health management and health education purposes. Except when required for counseling, teaching obligations, or legal provisions, all information will be kept strictly confidential in compliance with the Personal Data Protection Act.
